For a rational diagnostic approach to dry eye we need to evaluate both qualitative and quantitative disorders of the tear film and its distribution. That requires us to examine the eyelids with the meibomian glands, the tear film and the ocular surface. To check not only for neglected basic exams like blink rate and lid closure analysis but also for advanced tests like non-contact infrared meibography. To evaluate the tear film volume by matching, for example, Schirmer tear test and meniscometry; stability with interferometry and non-invasive break-up time; composition with osmolarity and lipid layer dynamic patterns. To examine the ocular surface not only with stains but also with Placido disk topography.
