CUMULATIVE ANTIBIOGRAMS AS DECISION SUPPORT FOR EMPIRICAL THERAPY

RESUMEN CORTO - SHORT SUMMARY:
Cumulative antibiograms are an important decision-making aid for the selection of an empirical therapy. They require at least 30 strains of a bacterial species from no more than one year. We isolated 121 strains of S. pseudintermedius in dogs from Italy and 61 strains in dogs from Spain in 2021. This fulfilled the criteria for the compilation of cumulative antibiograms, which we present here. Thus, we provide an up-to-date overview of the sensitivity of S. pseudintermedius in these two countries and support the practitioner in the possible selection of an empirical therapy. For Spain, however, it appears that many of the antibiotics tested do not achieve predictable sensitivity (> 80% sensitivity) against S. pseudintermedius. Practitioners in Spain should use an antimicrobial susceptibility testing (AST) for the treatment of S. pseudintermedius. The two antibiotics frequently used in dermatology, clindamycin and fusidic acid, have only a low sensitivity in both countries. In Italy, the following antibiotics showed good enough sensitivity to justify empirical therapy in dogs: amoxicillin/clavulanic acid, cephalexin, cefovecin, gentamicin, enrofloxacin, marbofloxacin, pradofloxacin, doxycycline, trimethoprim/sulfamethoxazole and nitrofurantoin. For Spain, this could only be shown for gentamicin, pradofloxacin, chloramphenicol and nitrofurantoin. Regardless of the result of the cumulative antibiogram, 3rd generation cephalosporins and fluoroquinolones should only be used if other antibiotics are ineffective according to the AST. This is part of the prudent use of antibiotics.
